Täiustatud protokoll Yarivi reagentide sünteesiks ja puhastamiseks

Emitsizumabi mõju APTT-le, FVIII testidele ja FVIII inhibiitorite testidele, kasutades erinevaid reaktiive: Ühendkuningriigi NEQAS-i tasemekatse tulemused

Sissejuhatus: Emitsizumab (Hemlibra: Roche Šveits) on humaniseeritud, kahespetsiifiline monoklonaalne modifitseeritud immunoglobuliin G4 (IgG4), mis seob inimese FX, FIX ja aktiveeritud FIX (FIXa) aktiivse FVIII aktiivsuse jäljendamiseks.

Eesmärk: Hinnake emitsizumabi toimet APTT-le, FVIII asendusaktiivsuse ja FVIII inhibiitori tulemustele.

Meetodid: Esitati kaks proovi, üks saadi emitsizumabiga ravitud raskelt hemofiilia A patsiendilt, kellel olid FVIII inhibiitorid, ja teine, mis saadi emitsizumabi in vitro lisamisega, kasutades plasma raskelt hemofiilia A patsiendilt, kellel ei olnud FVIII inhibiitoreid. Mõlemal proovil viidi osalevad keskused läbi APTT-ekraan, FVIII ja FVIII asendajate testid.

Tulemused: APTT tulemused jäid alla normi alumise piiri. Kromogeense FVIII testi tulemused Hyphen / Biopheni inimkomponendipõhise analüüsiga andsid oodatust kõrgema variatsioonikordaja (CV) tulemused, 38% -40%. Emitsizumabi kalibraatoritega modifitseeritud üheastmeline FVIII check näitas sarnaseid tulemusi sõltumata APTT reagendist. Inhibiitoranalüüsi mediaanproovid proovile S18: 23 = 1,43 BU (vahemik 0,9-3,Zero BU / ml, CV 38%). S18: 24 klassifitseeriti avastamise alampiiri alla.

Järeldus: APTT ekraanid näitasid järjepidevat lühenemist. VIII faktori modifitseerimata üheastmelise testi tulemused olid märkimisväärselt kõrged. APTT-põhised testid ei sobi emitsizumabiga ravitud patsientide hüübimisfaktorite või inhibiitorite mõõtmiseks.

Veiste päritolu kromogeensed analüüsid on emitsizumabi suhtes tundetud ja neid tuleks kasutada emitsizumabiga ravitud patsientide FVIII taseme / FVIII inhibiitorite jälgimiseks. Tulemuste varieeruvuse vähendamiseks tuleks kasutada tootespetsiifilisi kalibraatoreid.

Immunohistokeemiliste reaktiivide tuvastamine inimese kudedes koroonaviirusega seotud muutuste analüüsimiseks kohapeal valgu ekspressioonianalüüsiksUurisime kaubanduslikult saadavate monoklonaalsete antikehade (mAb) sobivust raske ägeda respiratoorse sündroomi koronaviiruse 2 (SARS-CoV2) immunohistokeemiliseks (IHS) tuvastamiseks standardsetes arhiiviproovides. Antikehad skriiniti HEK293 rakkudel, mis olid transfekteeritud viiruse nukleoproteiiniga, piigi valgu S1 ja S2 alaühikuga ning transfekteerimata rakkudel, samuti normaalse koe paneelil.

Kasutati ka kopsukudet, mille SARS-CoV2 olemasolu oli kinnitatud in situ hübridisatsiooni (ISH) abil. Testiti kokku 7 mAb: (1) mAb 001 (Sino Organic, 40143-R001), (2) mAb 007 (Sino Organic, 40150-R007), (3) mAb 019 (Sino Organic, 40143-R019), (4) mAb 1A9 (GeneTex, GTX632604), (5) mAb ABM19C9 (Abeomics, 10-10007), (6) FIPV3-70 (Santa Cruz, SC-65653) ja (7) mAb 6F10 (BioVision, A2060) . Spetsiifilist immunoreaktiivsust näitasid ainult 2 mAb-d, kloon 001 nukleoproteiinile ja kloon 1A9 S2 alaühiku piigivalgule.

Mõlemad kloonid näitasid COVID-19 kopsupõletiku ägedas faasis tugevat värvimist, enamasti ägeda difuusse alveolaarse kahjustuse piirkondades, kuid ei olnud täielikult kooskõlas. Viirusvalku leiti ka püsiva SARS-CoV2 infektsiooniga patsiendi neerutuubulites, mitme elundi endoteelias ja nina tampoonis.

Teised testitud reaktiivid olid kas halvasti reageerivad või näitasid mittespetsiifilist värvimist kudedes ja kahjustustes, mis ei olnud nakatunud SARS-CoV2-ga. Meie uuring näitab, et jäik spetsiifilisuse testimine on kohustuslik monoklonaalsete antikehade SARS-CoV2 hindamiseks ning kloonid 001 nukleoproteiiniks ja 1A9 kuni S2 alaühiku piigivalk on kasulikud SARS-CoV2 in situ tuvastamiseks.

Yarivi reaktiivid on glükokonjugaat-tris-asovärvid, mida kasutatakse laialdaselt taimebioloogias. Siinkohal kirjeldame optimeeritud protokolle, mida kasutatakse kuuest Yarivi reagendist koosneva paneeli sünteesimiseks, puhastamiseks ja iseloomustamiseks, ning soovitame lähenemisviise, mis võiksid olla kasulikud ka teiste glükokonjugaatide puhastamiseks ja iseloomustamiseks.

STA-NeoPTimal, ekstraheeriva tromboplastiini reagendi, mille ISI on 1 lähedal, hindamine
Sissejuhatus: Protrombiiniaeg (PT) on kõige nõutavam check kaasasündinud või omandatud koagulopaatiatega patsientide uurimiseks või suukaudse antikoagulantravi jälgimiseks. Tromboplastiinid võivad siiski näidata märkimisväärselt erinevat reageerimisvõimet Okay-vitamiini antagonisti (VKA) indutseeritud defektide suhtes ja neid iseloomustab seega nende ISI (rahvusvaheline tundlikkuse indeks).

INR-i tulemused on optimaalsed VKA-ga patsientide jaoks, kuid muudel põhjustel skriinitud patsientide jaoks võivad PT-tulemusi väljendavad suhted olla sobivamad. Kuna PT-tulemuste aruandlusüksust on PT-testimise taotluse põhjal väga raske määratleda, oleks ideaalne kasutada tromboplastiini ISI = 1. Uuringu eesmärk on võrrelda meie võrdlus-PT reaktiivi kahe tromboplastiinikandidaadiga, mille ISI on 1 lähedal.

Meetodid: Võrdlesime kolme erinevat tromplastiini: kahte küüliku ajuga ekstraheeritud reagenti (STA-Neoplastine CI Plus, ISI = 1,26, tavapäraselt kasutatav meie laboris ja STA-NeoPTimal, mille ISI = 1,01) ja rekombinantset tromboplastiini (STA-Neoplastine R koos ISI = 0,97). Võrdlus tehti 175 prooviga: 75 hüübimisdefektideta isikutelt ja 100 VKA alla kuuluvatelt patsientidelt.

Tulemused: STA-NeoPTimal ja STA-Neoplastine R korreleeruvad hästi meie referentsiga STA-Neoplastine CI Plus: regressioonivõrrandid on vastavalt y = 1,186x-0,1351, r2 = 0,9454 ja y = 1,1432x-0,1554, r2 = 0,9951 . Kõige väiksem kõrvalekalle INR-i tulemustest saadi STA-NeoPTimal reagendiga (intervall: -0,7 / + 0,4).

Järeldus: järeldame, et STA-NeoPTimalit saab kasutada laboris, kuna see annab tulemusi, mis on võrreldavad STA-Neoplastine CI Plus-ga. Lisaks tagab see tänu ISI = 1-le INR-iga võrdse PT-suhte teatamise, mis väldib vigu.

Α, α-asendamata krotüülboronaadi reaktiivide väljatöötamine ja stereoselektiivne krotüülimine Brønstedi või Lewise happekatalüüsi kaudu

• OEt2-katalüüsitud krotüülimisel, et saada (Z) -δ-borüül-sün-homoallüülalkoholid, millel on hea Z-selektiivsus. DFT arvutused määravad kindlaks kiraalse Brønstedi happega katalüüsitud asümmeetrilise allüülimise täheldatud enantio- ja stereoselektiivsuse lähtekohad. B-F3

• OEt2-katalüüsitud reaktsioonide stereokeemilised mudelid pakutakse Z-selektiivsete allüüllisandite ratsionaliseerimiseks. Want reaktsioonid loovad stereodefineeritud kolmeasendatud alkeeniühikuga väga väärtuslikke homoallüülalkoholi tooteid. Sünteetilist kasulikkust näitab lisaks salinipüroonide A ja B kogu süntees.

Reagendirakkude voolutsütomeetriline uuring ABO tüübi lahknevuse lahendamiseks

Eesmärgid: RBC alloantikehad võivad põhjustada ABO rühmitamise erinevusi, mis pole seotud A- või B-antigeenide või antikehadega, tekitades väljakutseid verepanga testimisel. Reaktiivrakkude immunofenotüüpide määramiseks ja kahe patsiendi ABO lahknevuste põhjuse selgitamiseks kasutati rutiinseid verepanga teste ja voolutsütomeetriat.

Meetodid: ABO lahknevus tuvastati kahel patsiendil pärast vereülekannet mitme RBC ühikuga. Mõlema patsiendi puhul näitas transfusiooni tüüp ja ekraan veregruppi A. Kaheksa ja 16 päeva hiljem näitasid mõlemad patsiendid näivat antikeha reaktiivirühma A rakkude suhtes, mis kutsus esile täiendava uuringu patsientide proovidega ja kaubanduslike reaktiivrakkude voolutsütomeetrilise testimise.

Tulemused: mõlema patsiendi proovis näitas transfusiooni hindamine Kell-antigeeni (Okay) tekkivat antikeha. Mõlema patsiendi RBC tüübid olid Okay negatiivsed ja mõlemad transfusiooniti Okay-positiivsete RBC-dega. Reaktiivi RBC voolutsütomeetriline analüüs näitas, et seitsmest partii numbrist viis olid Okay positiivsed.

Järeldused: Tekkivad anti-Okay antikehad viisid Okay-positiivsete reagent A1 rakkude aglutinatsioonini, tuues esile RBC alloantikehade ja reaktiivrakkude koostise arvestamise olulisuse juhtumite tõlgendamisel ilmse ABO rühmituse lahknevusega.
